Physical Forms & Information

• Sections A-E to be completed by parent/guardian

• Section F to be completed by a primary care physician

Sports Physical Section 7 Form - Re-Certification by Parent/Guardian

Section 7 to be completed by parent/guardian 1 Week Prior to participating in second or third sport. This form only needs to be completed if student-athlete plans to participate in a second or third sport and can only be filled out 1 week prior to participating in a second/third sport.  

Sports Physical Section 8 Form - Re-Certification by Licensed Physician of Medicine or Osteopathic Medicine Section 8 to be completed only if student-athlete sustains an injury throughout a sports season and is referred to a physician by the Athletic Trainer. This forms must be completed before the student-athlete can return to sports.
